Substitute Drivers

The Colonial Club Senior Activity Center is seeking an individual who enjoys working with older adults & making a difference in their lives.  This position will be primarily providing transportation for our Adult Day Center and those who attend our congregate meal site.  Additional duties include some transportation duties for general agency tasks.

Applicant must have: Current valid driver’s license and two years of experience driving vehicles through all types of weather. Experience working with various types of disabilities and working with older frail adults would be beneficial.  $13-$15 an hour starting pay, no evenings, weekends or holidays.

Substitute/Special Event Workers

Certified Nursing Assistants

Part time to full time position is available in the Adult Day Center and Supportive Home Care departments.

Adult Day Center position includes providing personal cares to older adults, leading activities, and daily housekeeping chores around the center.

Supportive Home Care position provides personal care and home chore services to older adults in their own home.

We are seeking an individual who possesses good communication skills, ability to work independently, dependable, and a certified nursing assistant is preferred. Travel within Northeast Dane County and a valid driver's license are required (reimbursement for mileage). Comparable compensation package is available. No nights, weekends, or holidays. Nutrition Coordinator

Full Time position is available in the Colonial Club Kitchen. The Nutrition Coordinator is responsible for the overall operations and management of the nutrition program. This includes congregate (onsite), home delivered meals and all contracted on site catering functions. Coordinate, organize and implement all aspects of daily meal service, including preparation of the meal, ordering of food and supplies and clean up. The Nutrition Coordinator also interacts daily with the older adult participants onsite and over the phone. This is a rewarding role in a fast paced environment.

Skills needed include basic computer knowledge, excellent interpersonal skills, experience in properly using commercial kitchen equipment, ServSafe training, organizational skills and ability to work with occasional interruptions. Dependability is key as meal preparation is Monday - Friday each day.

Case Manager

A part time position of 20 hours/week is available to provide case management services to older adults including information about services, programs and resources that assist them to experience daily life with dignity and security. Duties include conducting assessments and developing individualized service plans. Collaborate with community resources to ensure clients' needs are met and advocate for clients' rights and access to services, maximizing their opportunities for self-sufficiency.

Skill needed include a strong background in social work or related field, knowledge of older adult programs and related resources. Familiarity with Medicare, Section 8, and other public assistance programs is a plus.
