Making the Move to Facility Care: For Families Caring for a Loved One with Dementia

Thursdays, April 12 & 19 from 6:30 to 8:30 at Fitchburg Community Center, 5510 Lacy Road

These sessions are offered to help families make the transition from home to facility care.  For more

New Horizons

The Adult Day Center is one of several supportive services offered by Colonial Club. Professional staff members provide health care monitoring, personal care, social and recreational services. The program is available Monday through Friday from 8:00 am to 4:30 pm Participants can schedule as many days a week as they choose.

Service Planning

Each new participant receives an individualized assessment with input from the participant, their physician, the family, and concerned others. A service plan is designed to respond to the needs of each client.
