This service is provided to persons over the age of 60 living in their own home or with a family member or friend and are in application for or currently on Medical Assistance (Title 19).  We have a case manager who will provide such services as coordination of services required to keep the individual in the home/ independent in the community as long as possible, supplying resources for informed decision making, educate individuals to make informed choices, and complete comprehensive assessments for all of our active clients.  This case manager will also assist you with your application and other necessary documents for the Medical Assistance program. We accept referrals from discharge planners from nursing homes and hospitals, physicians and clinics, home health agencies, social workers, local police departments and you.  As with our other services we serve northeast Dane county.
