Case Management

     
 

This service is provided to persons over the age of 60 living in their own home or with a family member or friend.  We have 3 case managers who will provide such services as coordination of services required to keep the individual in the home/ independent in the community as long as possible, supplying resources for informed decision making, educate individuals to make informed choices, and complete comprehensive assessments for all of our active clients.  We accept referrals from discharge planners from nursing homes and hospitals, physicians and clinics, home health agencies, social workers, local police departments and you.  As with our other services we serve northeast Dane county.  Currently there is not a fee associated with this service, however contributions are always welcomed.
